Intensive Home Based and Outreach Services
This program provides a multidisciplinary team approach to treating children and their families in Southeastern Massachusetts and Rhode Island. Each family receives a full evaluation and assessment of needs, which becomes the focal point for change during the program’s intervention. The team provides intensive, short-term, home-based treatment and case management services to children, adolescents and their families in an effort to prevent repeated admission to psychiatric hospitals, residential facilities, or foster care placements. The team works to stabilize children and adolescents with mental health, child protective and substance abuse issues.

Healthy Families Massachusetts is a home visiting program available for first-time parents age 20 and younger, funded by the Children’s Trust Fund. Our home visitor gets to know the parents and their baby; and helps them build their parenting skills, answers their questions and works with them to strengthen relationships. The home visitor helps the family set goals and find the right services and programs for young parents in the community; including day care, health insurance, job placement agencies and continuing education.

Total Achievement Program (TAP)
The TAP program serves youth who have social or emotional disabilities; and are in need of consistent, intensive, therapeutic support to maintain their current placements at home, in a foster home, and in school. A therapeutic milieu program, provided after school and during school vacations, focuses upon building positive peer relationships and managing behaviors in a group setting. Ages served are 6-16 years old, depending on the site. The Hyannis TAP offers a Teen Group on Friday directed toward the development of social skills and independent living skills. Individual and family counseling are offered in homes, as well as in the office. As part of a comprehensive treatment team, counselors work closely with the schools and community agencies to support each client’s educational success. We offer a therapeutic summer day camp for 7-8 weeks, depending on the particular site, in collaboration with YMCA.
